New Patient Visit

Service Description

A non-refundable $150 deposit is required to hold all new patient appointments. The deposit will be applied to the cost of the new visit. Payment is required at the time of booking using our credit card processor. Please complete the necessary paperwork under the "New Patient Information" tab at the top of the website before your visit.

Cancellation Policy

PLEASE READ: Change/Cancel Appointments: To cancel or change appointments you MUST go to the scheduling web-page and create a login and password. Found here: Click on the "login" button and then create a username and password. If you call the office to cancel or change the appointment you may still get an appointment reminder. Cancellation Policy: There will be a $85 dollar cancellation fee for appointments not canceled within 24 hours. In the event of three (3) documented “same-day cancellations,” the patient will be subject to dismissal. New patient no shows will be dismissed from care and charged $85 dollar cancellation fee.
